Liability Release and Agreement

By registering for and participating in the Spring Break Baseball Camp (“Camp”), you acknowledge and agree to the following terms and conditions:
1. Release of Liability
Participation in the Camp involves physical activities that may result in injuries, including but not limited to sprains, strains, fractures, and other physical harm. By enrolling in the Camp, you acknowledge that such risks are inherent to sports activities and fully accept responsibility for any injuries or damages that may occur.
You hereby release and discharge JL Baseball, its directors, officers, coaches, staff, volunteers, and affiliates from any and all claims, liabilities, or damages arising from participation in the Camp. This release applies regardless of negligence and includes injuries caused by actions or inactions of the Camp staff.
2. Assumption of Risk
You understand and agree that participation in the Camp is entirely voluntary. You assume full responsibility for any injuries or damages sustained during Camp activities, including but not limited to practices, games, drills, and other related events.
It is the responsibility of the participant’s parent or legal guardian to maintain adequate medical insurance coverage for any potential injuries.
3. Medical Treatment Authorization
In the event of a medical emergency, you authorize JL Baseball staff to arrange for emergency medical treatment as deemed necessary for the participant. All associated costs for medical treatment will be the sole responsibility of the participant’s parent or legal guardian.
4. Photography and Media Consent
By participating in the Camp, you grant JL Baseball permission to take photographs and videos of the participant during activities. These images and recordings may be used for promotional purposes, including social media, website content, and other advertising materials.
If you do not wish for your child’s image to be used, you must notify JL Baseball in writing before the start of the Camp.
5. Dispute Resolution
Any disputes arising out of or related to participation in the Camp will be resolved exclusively through mediation. Mediation will take place in [Insert Location] with a mediator mutually agreed upon by both parties. The cost of mediation will be shared equally.
Both parties waive the right to pursue legal action in court, including lawsuits, except to enforce a mediation agreement.
6. Acknowledgment
By completing registration, you confirm that:
– You are the parent or legal guardian of the participant.
– You have read, understood, and voluntarily agree to this Liability Release and Agreement.
– You accept all terms on behalf of yourself and the participant.
